Licensing and Certification

If you wish to file a complaint about a registered or certified counselor or a licensed or certified alcohol or other drug service provider, please complete the Complaint Form and submit to ADP at the address or fax listed below. Additional information regarding filing a complaint is contained in the Toll Free Complaint Number and Provider Responsibility to Report Counselor Misconduct Letter (Dated March 24, 2010).
